Regular Patient (RP) - These patients may come and go as they please during the day from their rooms and in the common rooms, and outside. (Rooms 01-30)
Lockdown (LD) - These patients are placed in cells that are monitored on a schedule and rarely let out. When they are let out it is closely supervised with limited contact with other patients. (Rooms 31-53)
Suicide Watch (SW) - These patients get out the second most, but are closely supervised outside of their rooms. Their rooms are designed so that there is no way for them to harm themselves, and have regular room checks. (Rooms 54-76)
Padded Rooms (PR) - The patients in the padded rooms are kept in straight jackets all the time, and have no furniture that they can harm themselves on. Occasionally they are taken out for excercise and cleaning, but rarely are they let out of their straight jackets. (Rooms 77-99)
Of course anyone who is not a patient, the room and patient number won't matter, but remember to mention what they specialize with if they are a doctor.
With the format, once again go to Edit > Replace or press CTRL+H, then replace < with [ and > with ].
